What is Hospice and Palliative Care?
If you or someone you love is living with a life-limiting illness, we can provide you with a full program of support.
What is palliative care?
Palliative care is the prevention and relief of suffering by a specialized medical team that focuses on managing symptoms, relieving pain, and improving quality of life, whatever the diagnosis.
What is hospice care?
A form of palliative care for patients with a life expectancy of six months or less.
Who pays for hospice care?
Medicare, Medi-Cal, TRICARE, HMOs, private insurance providers, individuals and philanthropic contritbutions.
How is Hospice Care provided?
With interdisciplinary teams of specialists focused on your needs—medical, psychological, family-related, and spiritual. Our care teams can include a nurse, social worker, chaplain, home health aide, and supervising hospice physician. We take a wide-ranging approach to giving both patients and their families the best possible quality of life, for as long as life lasts.
Where is care received?
We provide care wherever it works best for the patient. While most people prefer staying at home in familiar surroundings, we also provide care in community care centers, hospitals, residential care and skilled nursing facilities. We care for over 1,000 patients a day all over San Diego County, from Camp Pendleton down to the Otay Mesa border.
We have staffed phone support available 24 hours a day, 7 days a week, with medical clinicians on call around the clock. We also have resources to help find solutions for legal and financial issues regarding hospice care.
How does Hospice Care begin?
Patients of all ages may be appropriate for hospice care once eligibility criteria are met. To explore options, we will meet with patients and their loved ones at no charge to discuss possible hospice care. Please call and ask to speak to an access expert. The call is toll-free. 866-688-1600.
